critical NHTSA 15V284000 May 13, 2015

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2004-2005 Toyota RAV4 vehicles manufactured May 13, 2003, to October 25, 2005

In the event of a crash necessitating deployment of the driver's frontal air bag, the inflator could rupture with metal fragments striking the driver or other occupants resulting in serious injury or death.

Fix: Owners of these vehicles are advised not to drive their vehicles until the recall remedy has been completed. Toyota will notify owners, and dealers will replace the driver's frontal air bag, free of charge. Owners were mailed an interim notice June 17, 2015. The recall began on August 21, 2015. Owners may contact Toyota customer service at 1-800-331-4331.

Is there a fix? Manufacturer service bulletins

The manufacturer has issued service bulletins covering airbags on this vehicle — documented repair instructions, service campaigns, or warranty extensions sent to dealers. A TSB isn't a recall (it's not a free safety remedy), but it's the manufacturer acknowledging the issue and how to fix it.

The failure pattern owners describe

The dominant issue here is a Takata driver-side airbag inflator defect (NHTSA campaign 15V-284) that threatened rupture during deployment and could send metal fragments through the cabin. Starting in May 2015, Toyota issued interim notices saying no remedy was available yet. When recall parts finally came, the supply chain collapsed—dozens of owners report parts on backorder for 6 months, a year, or longer. One dealer would only order one part per week; others never called back. Owners couldn't schedule repairs, couldn't get the loaner cars the recall notice promised them, and had no way to know when (or if) the work would happen.

When replacements did arrive, Toyota often installed another Takata unit—the same brand Toyota said it had sourced alternatives for. Several owners reported passenger-side airbags (identical to the recalled driver unit) were never included in the recall despite being the same defective design.

Separately, multiple owners experienced airbag non-deployment during real crashes—head-on collisions, rollovers at high speed, 30–40 mph impacts—with no deployment and serious injuries. A dealership blamed the impact angle. And one vehicle developed an airbag warning light, horn failure, and cruise control failure at 244,000 miles; the dealer said the spiral core needed replacement.

The administrative mess was equally severe: completed recalls didn't update in any system; vehicles stayed flagged as non-compliant even after repair; one owner received harassment notices years after trading the car in; DMV and insurance couldn't clear the flags. Toyota customer service couldn't answer basic questions about timing or status.

Failure modes owners describe

Takata Driver-Side Airbag Inflator Defect

Driver-side airbag inflators supplied by Takata are susceptible to rupture during deployment, potentially creating metal fragments that can injure or kill occupants. The defect involves moisture intrusion making inflators more susceptible to rupture in a crash. NHTSA campaign 15V-284 recalls driver-side inflators in 2005 RAV4s.

When: Campaign 15V-284; notices issued May–July 2015

Symptoms owners cite: No external warning light or indication of the defect before a crash; Airbag inflator susceptible to rupture upon deployment; Risk of metal fragment ejection on impact

Codes mentioned: 15V284000

Repairs/costs cited: Replacement inflators were initially Takata units, sometimes replaced with non-Takata units. Parts availability was severely delayed; many dealers had parts on backorder for months (7+ months reported). One dealer reported ordering only one part per week for RAV4 repairs. Repair completion took weeks to months after recall notice issued.

Recalls/TSBs owners mention: Recall campaign 15V-284; interim notices (May 2015) stated no remedy available. Toyota issued second notice when remedy available (July 2015). Toyota promised loaner vehicles per recall terms but dealerships often refused to provide them if airbag light was not illuminated. Toyota documented plan to secure non-Takata inflators but many replacement inflators supplied were also Takata units. Passenger-side airbags (same design as recalled driver inflators) were not included in recall despite being identical to defective units.

Airbag Non-Deployment During Crashes

Airbags failed to deploy in multiple vehicles involved in significant collisions. One vehicle hit at 30 mph resulted in severe injuries; another flipped and skidded 600 feet on its roof with no deployment; a head-on collision with $8,000+ damage produced no deployment. A dealership claimed airbags don't always deploy depending on impact angle.

When: Incidents reported 2014–2018; failure mileage ranging from 73,000 to 178,000 miles

Symptoms owners cite: No airbag deployment in head-on collisions; No deployment in rollover at high speed (100+ mph); No deployment in moderate-speed impacts (30–40 mph)

Codes mentioned: 15V284000, 06V096000

Repairs/costs cited: Vehicles were not repaired before failure. Post-crash, one vehicle was towed to independent facility; another to body shop. No diagnostic work performed at time of complaint filing.

Recalls/TSBs owners mention: One case: manufacturer stated vehicle was not included in campaign 06V096000. Another case: dealership told owner airbags don't always deploy depending on impact direction (17-degree angle criterion mentioned). No recall action or field fix documented for these specific non-deployment failures.

Airbag Warning Light and Spiral Core Malfunction

Driver-side airbag warning light illuminated during normal vehicle startup. Concurrent failure of horn and cruise control, indicating a common electrical circuit issue (the spiral core in the steering column).

When: At 244,000 miles

Symptoms owners cite: Airbag warning light illuminated; Horn failure; Cruise control failure

Repairs/costs cited: Dealership technician identified spiral core (steering column contact assembly) as needing replacement. Vehicle was not repaired; no parts cost or labor data provided.

Recall Parts Supply Chain Failure

Severe and prolonged shortage of replacement inflator parts for recall 15V-284. Approximately 40 complaints (narratives #15–50 and others) report dealers unable to source parts, delays extending 6 months to over a year, and inconsistent supply allocation.

When: October 2015 through early 2016 and beyond

Symptoms owners cite: Parts on backorder indefinitely; Dealers unable to provide scheduling or estimated completion dates; Consumers unable to get recall remedied despite notification; Island and remote location vehicles unable to access certified technicians; One dealer restricted to ordering one part per week

Codes mentioned: 15V284000

Repairs/costs cited: Multiple dealers reported parts unavailable: Conklin Motors (Kansas), AutoNation Toyota Corpus Christi, Principle Toyota (Memphis), Boch Toyota South, and many unnamed dealers. Some consumers waited months without callback; others were told to call back weekly.

Recalls/TSBs owners mention: Toyota issued interim notices but no remedy available initially. When remedy was announced, parts were not supplied to dealers in sufficient quantity. Some consumers requesting loaner vehicles per recall terms were denied. Island residents were told vehicle would need to be barged to another island and that consumers were responsible for transport costs. System updates failed to reflect completed recalls.

Dealership and Toyota Customer Service Non-Compliance

Dealerships and Toyota customer service repeatedly failed to honor recall terms, provide timely communication, or supply loaner vehicles. Some dealerships stonewalled owners; others made upselling attempts during recall work.

When: 2015–2016

Symptoms owners cite: Dealerships denying rental cars despite recall terms promising them; Voice mail callbacks never returned; Dealerships claiming they would only call when parts arrived, then not calling; Dealership pushing $1,000+ in unrelated repairs at time of recall visit; No second notification letter from Toyota as promised

Repairs/costs cited: One dealership (Passport Toyota, Suitland MD) and another (DarCars Toyota, Silver Spring MD) refused rental cars. Larry Miller Toyota told customer repair would take 1.5 hours but then claimed part had to be ordered and attempted to sell unrelated repairs.

Recalls/TSBs owners mention: Recall notice stated dealerships must provide rental vehicles if owner felt unsafe; Toyota customer service unable to provide status or timeframes. Toyota failed to send second notification letter as promised in interim notice. DMV and insurance records not updated after recall completion, leaving vehicles flagged as non-compliant.

Recall Database and Administrative Failures

Vehicle recall status not accurately reflected in NHTSA and Toyota systems; completed repairs not recorded; VIN not properly associated with recalls; notification letters continued to previous owners.

When: 2015–2018

Symptoms owners cite: Recall status showing 'incomplete' weeks after dealership reported repair finished; Vehicle showing on recall list after repair completed (7/15/25 repair still showing as outstanding); VIN not associated with recall despite owner receiving notice; No second letter from Toyota explaining remedy; Continued recall notices sent to previous owner who traded vehicle in (October 2014) and notified Toyota multiple times

Codes mentioned: 15V284000

Recalls/TSBs owners mention: Toyota and dealership unable to update systems; DMV unable to receive updates; insurance companies unable to clear flags. Multiple system syncing failures between Toyota corporate, dealerships, and NHTSA.

Common questions

How serious is the airbags problem on the 2005 Toyota RAV4?

It's a meaningful issue. 58 complaints have been filed and the failure mode causes operational problems for owners. Repairs average $1,100.

At what mileage does the airbags typically fail?

Across the 16 complaints that reported odometer mileage, most airbags failures cluster between 48,000 and 166,000 miles, with the median around 110,000. A quarter of owners report trouble before 48,000; a quarter make it past 166,000. Maintenance history matters more than the odometer alone — this is the reported failure window, not a guarantee.

What does it cost to fix?

Independent shops typically charge around $1,100 for airbags repairs on this vehicle. Dealer pricing tends to run 20-40% higher. The exact figure depends on the specific failure mode, parts availability, and your local labor rates. If you're outside factory warranty, an extended service contract often covers this category.
