The failure pattern owners describe

Buyer takeaway: This 2021 RAV4 has multiple steering system issues reported by owners: Lane Keep Assist malfunctions that jerk the car across lanes unexpectedly, power steering low-voltage warnings causing shutdowns and wheel lockup at highway speeds, and occasional complete steering wheel seizure. Some owners have been in accidents or near-crashes. Dealers often cannot diagnose or replicate these problems, and Toyota recalls exist for some vehicles but not all affected models.

Owners of 2021 RAV4s describe two main steering system failure patterns.

Lane Keep Assist malfunction: The Lane Centering and Lane Keep Assist systems activate unexpectedly and steer the vehicle hard left or right—sometimes the opposite of intended function. On highways at 45–65 mph, owners report violent side-to-side swaying across lane lines, steering into oncoming traffic, or off the road shoulder. The system has forced cars toward cement trucks, into ravine embankments with reflector signs, and down mountainsides resulting in rollover. One owner reported the system steered her daughter's car into a parked vehicle at 23 mph. Dealers cannot replicate the issue or locate a fault. Toyota customer service initially claimed everything works normally but later acknowledged a Technical Bulletin (2019–2021) addressing steering hijacking existed.

Power steering voltage failures and shutdowns: Vehicle displays "Low Voltage Power Steering" or "Voltage Abnormality" warnings, typically after auto idle-stop engages at traffic lights or while parked. In severe cases, the warning precedes complete loss of power steering and engine shutdown at highway speeds—steering wheel locks solid, becoming nearly impossible to maneuver. This has occurred at 35,000–42,000 miles and beyond. One owner experienced shutdown at 65 mph in freezing, remote conditions. Dealers test alternators, starters, and batteries (all normal) but cannot diagnose the root cause. One case was traced to corrosion in the high-voltage floor harness. Toyota recalls exist for this issue on some VINs but not others, leaving many owners unaware and unrepaired.

Steering wheel seizure and rack-and-pinion stiffness are also reported but less frequently documented.

Failure modes owners describe

Lane Keep Assist / Lane Centering malfunction

Lane Keep Assist and Lane Centering systems activate erratically, steering the vehicle hard left or right across lane lines, into opposing traffic, off the road shoulder, or into stationary objects. The system sometimes does the opposite of its intended function. In some cases, owners report the system makes violent, side-to-side swaying motions across lanes. Dealers have been unable to replicate the issue or cannot identify the root cause.

When: Early ownership; most complaints around highway speeds (45–65 mph). One incident reported at 23 mph during a turn.

Symptoms owners cite: Steering wheel turns hard 15–20 degrees unexpectedly; Vehicle veers left or right across lane lines; Violent swaying back and forth across lanes; System steers car into oncoming traffic or off road shoulder; Lane Departure Alert alarm triggers or beeps in response; No warning lights prior to malfunction in most cases

Codes mentioned: Lane Departure Alert alarm

Repairs/costs cited: Dealers unable to replicate or diagnose. No repairs documented. One owner mentioned a past Technical Bulletin (2019–2021) released by Toyota addressing this steering hijacking issue.

Recalls/TSBs owners mention: Toyota customer service and dealership service departments stated vehicle settings were normal and everything working properly. One dealer consulted with Toyota and was told the same. One owner reports a Toyota Tech Bulletin (2019–2021) exists for this issue but was not proactively communicated.

Power Steering Low Voltage warning and unexpected shutdowns

Vehicle displays 'Low Voltage Power Steering' or 'Voltage Abnormality: Power Steering Low' warning message. In severe cases, the warning precedes a complete loss of power steering control and engine shutdown while driving, sometimes at highway speeds. The vehicle enters a fail-safe mode and the steering wheel locks, becoming extremely difficult to maneuver. In other cases, the warning occurs while idling at traffic lights or parked. Owners report the issue is intermittent and correlates with the vehicle's automatic idle-stop feature.

When: Occurs at 7,500 miles, 27,000 miles, 33,000 miles, 42,000 miles, and at 97,000 miles. Most commonly reported between 35,000–42,000 miles. Happens during highway driving, at idle/traffic lights, or while parked.

Symptoms owners cite: Warning message 'Low Voltage Power Steering' or 'Voltage Abnormality: Power Steering Low'; Steering wheel locks during highway driving; Sudden loss of motive power on highway at 65 mph; 'Hybrid System Malfunction' warning illuminates; Vehicle completely shuts down and becomes undriveable; Difficulty maneuvering to shoulder in remote or dark areas; Warning appears after automatic idle-stop feature engages; All dashboard lights illuminate; Message 'Parking Hold Unavailable' appears; Vehicle unable to restart immediately

Codes mentioned: Low Voltage Power Steering, Voltage Abnormality: Power Steering Low, Hybrid System Malfunction, Check Engine

Repairs/costs cited: One case involved replacement of High-Voltage Floor Under Wire Harness (MGR Cable) and installation of a splash guard cap due to severe corrosion. In other cases, battery was tested and recharged (outputting low voltage); alternator and starter tested normal. Most owners report dealers are unable to diagnose the issue or find any electrical fault. One dealer charged a diagnostic fee but admitted inability to perform the service.

Recalls/TSBs owners mention: Owners report Toyota has issued recalls for power steering voltage issues on certain 2021 RAV4 models, but not all affected vehicles are included. One owner states a recall exists but not on their VIN. Multiple owners mention seeing recalls online for this issue but their vehicle was not covered. One dealership initially dismissed the complaint as normal operation before reviewing a tech bulletin.

Steering wheel seizure / Loss of electric power steering

Steering wheel suddenly becomes locked or seized during normal driving, rendering the vehicle extremely difficult or impossible to steer. One case occurred during a turn at 23 mph; another while driving at an undisclosed speed attempting a right turn. Owners report no warning lights prior to failure in most cases.

When: One incident at 23 mph during a turn; one at undisclosed speed during a right turn; one incident at 97,000 miles.

Symptoms owners cite: Steering wheel suddenly seizes or locks; Steering becomes extremely stiff and difficult to turn; Loss of electric power steering (EPS); Vehicle difficult to maneuver to safety; No warning lights in most cases

Codes mentioned: Steering angle sensor fault (mentioned in one case)

Repairs/costs cited: One vehicle collided with a street sign due to loss of EPS. Another sideswiped a parked vehicle. In both cases, dealers were unable to diagnose or repair the vehicles. One case mentioned a faulty steering angle sensor but was not repaired.

Recalls/TSBs owners mention: One manufacturer informed the owner that a certified Toyota technician found no fault with the vehicle. In other cases, no manufacturer response documented.

Steering angle sensor fault

Steering angle sensor malfunction causes warning display (steering wheel with exclamation mark) to illuminate on the vehicle screen. Warning blinks and then turns off.

When: Approximately 27,000 miles.

Symptoms owners cite: Screen displays steering wheel icon with exclamation mark; Warning light blinks then turns off

Codes mentioned: Steering angle sensor fault

Repairs/costs cited: Diagnosed by dealer but not repaired.

Water intrusion and flood damage following repair

Vehicle rear floor became completely flooded with water. After Toyota service detected water, they kept the vehicle 3 weeks and performed major repairs, which they stated would be fully covered. Upon pickup, the owner immediately experienced 'Low Voltage Power Steering' warning and steering wheel lockup. Vehicle returned for further diagnosis and flooded again after repair completion.

When: April–May 2023; one year after purchase.

Symptoms owners cite: Rear floor of vehicle completely flooded with water; Bad chemical smell in car; Water persists after initial service attempt; Flooding recurs after major repair work

Repairs/costs cited: Toyota kept vehicle 3 weeks for repairs; stated damage would be fully covered. Source of leak not initially identified. Upon pickup, steering wheel locked and 'Low Voltage Power Steering' warning appeared.

Recalls/TSBs owners mention: Owner discovered a recall that had never been communicated to them. After repair, the vehicle flooded again, and Toyota apparently wanted the owner to pay for further service.

Rack and Pinion stiffness and noise

Steering rack and pinion system exhibits stiffness and noisy operation. Diagnostic from Toyota dealer indicated rack and pinion damage.

When: Timing not specified.

Symptoms owners cite: Stiff steering; Noisy steering operation

Repairs/costs cited: Toyota dealer recommended replacement of rack and pinion assembly.
