The brake system is constantly failing/malfunctioning.It is available for inspection upon request. There has been several incidents where other vehicles have nearly been rear ended. I took my 2011 Toyota Prius (hybrid) to Toyota approximately 8 months ago. They/Toyota did confirm that the problem was indeed the brake booster. The warning lamps did come on soon as the brakes started malfunctioning.

I have a 2011 Toyota Prius with 85K miles, as I was driving on the highway the abs light, brake light, and TSV light came on and then brakes were not working like normal. Had to pump the brakes very rapidly and steer off to the right side of the highway and use the emergency brake to come to a complete stop. I called the Toyota dealer and they said its the brake accumulator fault and they will not cover the fix because it was out of the 10year/150K mile warranty per SB. Towed my car to an independent service center and they confirmed it was the brake accumulator C1391 code. cost me $3000 to fix.
ABS, Slip Indicator and Brake warning lights have appeared on the cars dashboard. DTC fault code C1391 has appeared indicating a failure of the Brake Booster and Brake Booster Pump assemblies per Toyota Customer Support Bulletin (ZJB) No.: POL18-03. Loss of regenerative braking, putting the safety off occupants at risk.
The brake system appears to break down; ABS module and brake booster lose power. This causes braking to be harder and potential safety for the vehicle to lose all braking capability. This issue occurs when braking. There appears to be less air pressure in assist in the braking. Reading online this is a very common issue for the 2011 vehicle! All brake lights on Dashboard came on. Almost got into an accident because brakes were not responding. Brakes started responding but lights remained on. At time it will contibue to beep and Brakes do not respond. I am seeing that this is a common issue happening to most Prius vehicles after a certain period. It is extremely expensive to repair. Since this is happening on quite a lot of Prius vehicles, I feel this should still be covered by Toyota. This appears to be a faulty Brake Pump and Brake Booster. This has not been inspected by a dealer but being that the car is a 2011, I'm afraid they will decline from what I have been reading.

The problem my car experienced happened while the vehicle was in motion on I-110 in pensacola, florida and presented itself abruptly (through the car's warning indicators at least) on friday, august 31st headed to pensacola beach for labor day weekend from our home in baton rouge, louisiana. All the car's brake warning lights came on simultaneously and without warning, the master brake light, the ABS light, five different lights in all. I pulled off the interstate, went to an autozone store, checked the brake fluid level and brake pads (all looked nominal) and got the car's computer checked for warning codes (none came up). I was traveling down a mountainous road and the road surface changed from gravel to asphalt. I was also braking at the same time when the brakes seem to become nonexistent and the car just lurched forward. This has happened 3 times before. *tr
Takata recall my 2011 prius has an issue where the brake becomes spongy at random while driving under normal conditions. The brake, ABS, and traction control lights all illuminate simultaneously. After resetting the computer, the issue comes back within 10-30 minutes of driving at random. Toyota told me it is a $3,000 fix and there is no recall for my year. My car has 110,393 miles.
The Master Cylinder started failing while I was driving the car, the car only has 138k miles, the dashboard lit up like a Christmas tree, all the warning lights were on, and when I went to step on the brake, I had to push it all the way to the floor for any braking to happen. I remember being scared for my life because I had almost no brakes which I could've got into a serious accident or been killed had it failed completely. I took the car to the dealer and they diagnosed it with code C139: Abnormal Leak at Accumulator Assembly, and recommended a new Brake Booster with Master Cylinder Assembly and Brake Booster Pump Assembly with Bracket. The car by age was not in Toyota's extended warranty on the parts but mileage-wise it still is, so I was footed with a $4-5k bill to replace both parts.

High intensity discharge (HID) bulb in front passenger side headlight went out. No explanation for why this occurred; it simply burned out. These bulbs should last a lot longer than 26,000 miles. *tr

Both head lights stopped working at the same time, only high beam will work, regular beam stopped working and will not come back on. *tr
I have had this car since 2021 and have had to replace both the high and low beams multiple times a year. Each time they go out at the same time with no warning. This is deadly as I have. been on the freeway at night and almost crashed. I counted 4 times just in 2023 this has happened to me. This is more then any other car ive owned, along with any car my siblings and parents have owned. This is deadly and needs to be recalled ASAP

I have reported this once and numerous times to Toyota. I even paid to "fix" the problem. My daughter was driving and all the dashboard lights came on and the brakes do not work. It's doing it again after they supposedly "fixed" it. My family is at risk! If anything happens, I will seek legal action. I can't believe a company as big as Toyota wont take responsibility. And that a recall wont be done. I've seen numerous people report the same issue. PLEASE do something.

The car suddenly decelerated from 65 miles an hour to about 5 miles per hour while driving on a highway at night with the shoulder blocked due to road construction. This same problem was present in the 2004 prius and was apparently the subject of NHTSA investigation. There was a recall issued on november 30, 2010 for 650,000 units of the 2010 prius. Toyota has not been able to determine the cause of this problem in the earlier models and admits it doesn't know the cause of the problem in the 2011 prius. This defect presents a great danger, especially when driving in heavy traffic at highway speeds. *kb

Tl- the contact owns a 2011 Toyota prius. The contact stated that the side windows would become extremely foggy and the contact was unable to utilize the side-view mirrors when needed. The contact would have to open the windows in an attempt to clear the windows but to no avail. The contact spoke with the manufacturer and was advised that the failure was not considered a safety defect and that they would be unable to assist. The failure mileage was 5 and the current mileage was 6,000. Kmj - updated 02/09/12*lj the consumer was informed it was a design problem and nothing could be done about it. The consumer also stated the left side view mirror vibrated at high speeds. The dealer found the mirror was losing its housing. Updated 02/27/12
