12/21/15 - my vehicle was stationary. I just finished refueling and started the car. I had my right foot on the brake, and switched from park to drive. My left foot stayed on the floor, to the left of the brake pedal. Without removing my right foot from the brake, the engine revved and the car lurched forward. I pressed extremely hard on the brake to keep the car from moving forward, and could hear and feel the engine and brake fighting each other. This lasted approximately 3 seconds and the engine returned to idle. The brake and accelerator pedals are not that close, it would be impossible to press both at the same time.

Tl* the contact owns a 2012 Toyota rav4 EV. The contact stated that the vehicle was taken to the dealer for a manufacturer's recall remedy. Five days after the recall remedy was performed, the contact experienced unintended acceleration. While driving in a parking lot at 5 MPH, the vehicle surged forward to a high speed without warning. The vehicle crashed over a median, a concrete barrier, and into a parked vehicle. The driver sustained minor whiplash, but no medical attention was required. A police report was not filed. The vehicle was towed to a dealer and the contact waited over three weeks for the manufacturer's engineer to provide an analysis on how the failure occurred. The approximate failure mileage was 50,000.

On thursday 18 december, my wife and I left springfield, missouri to dallas, texas to visit our family for the christmas holiday. We were driving our recently purchased 2012 rav4 with approximately 33,000 miles which we purchased in late september 2014. We stopped several times during our trip to refuel and allow my wife, who is 6 month pregnant, a chance to stretch her legs. For much of the trip, I used cruise control when possible at varying speeds from 65 MPH (in missouri) to 75 MPH (in parts of oklahoma). At approximately 4:30 pm, while driving through tushka, oklahoma we approached a stoplight where the speed limit dropped from 65 to 45 MPH. Inaudible turn signal prompt: Older drivers who drive for miles with their turn signal on is the subject of many a cartoon and common source of highway frustration. It is also a safety issue when a vehicle does not perform the turn that it is apparently signaling to make. I find myself being one of those older drivers who leaves their turn signal on. I have moderate hearing loss and do wear hearing aids, but not all the time. Perhaps my RAV4's audible prompt is quieter than others, but with road noise and the radio on I often do not hear it. A simple solution to this problem would be the ability to control the volume of the audible turn signal prompt so it could be "cranked up" to be easily heard. Poor winter visibility: This is an issue with this vehicle but also with almost all other vehicles with internal combustion engines. During cold weather the defroster is ineffective until the engine warms up and provides heat. This results in two unsafe conditions 1) warming the engine up while parked, which exposes the vehicle and driver to theft ("carjacking" is an increasing crime in our area) and increases carbon emissions, or, 2) driving the vehicle while the engine is cold and having poor forward visibility. If defrosters were equipped with electric heating elements, defroster heat would be provided almost immediately. I was told that VW provided this feature for several years and it was very effective. Both front drive axles exhibited signs of severe corrosion damage leading to complete loss of lubricant and had to be replaced at 34,056 miles. The vehicle is garaged and not subject to any unusual operating conditions. Symptoms included severe cracking and popping noises on turns.

On october 17 shortly before 2 pm pst in clear weather, I was pulling into a parking spot on private property at about 2 to 3 MPH. I turned the vehicle into the spot and applied the brakes but the car surged over the curb, hit a tree, went over an embankment and crashed onto a parked car. (the airbag did not deploy, but there were no injuries.) the ignition would not turn off after repeated attempts. Finally I depressed the ignition button for about 10 seconds and it turned off. Just 5 days earlier, the car had undergone a safety recall repair to the electronic circuitry. Toyota had issued a safety recall due to a software defect which affected "components in the electric vehicle traction motor assembly." we informed Toyota this appeared to be an unintended acceleration incident. While driving 35 on a local road, the vehicle lost all power to the propulsion system. Power steering and brakes worked as I was lucky to be able to coast safely to a turnout. The vehicle warning light saying "check EV system, have dealer check immediately" was illuminated on the information display. This is a common problem for the rav4 EV as the Toyota and Tesla parts have trouble maintaining communication with each other within a part called the gateway ECM. My car was serviced by the dealer after this incident, where they updated the firmware loaded on the gateway ECM.

The mechanism that controls the up/down position of the sun visors breaks after a few years of use. As a result, the sun visors will not stay in a 'closed' position (against vehicle roof) and constantly fall halfway down obstructing proper view of the road. The drivers side sun visor broke several years ago. When I took it to the Toyota dealer they stated the only way to fix this was to replace the visor (at my expense). The cost to replace one visor was >$200. The passenger side visor has now failed to hold in place as well. Toyota assumes no responsibility for this design defect (plastic part in the visor mechanism wears or breaks). This is a safety issue due to poor design on Toyota's part. Consumers should not have to pay $200 to have these replaced.

I noticed that the 'pass air bag off' light was on even though a passenger was sitting in the front passenger seat. I took it to the Toyota dealership that diagnosed it. They said the sensor in the passenger seat belt buckle had gone bad and needed replacing. What bothers me is the fact that the airbag light on the instrument panel didn't come on to indicate that something was wrong. I am not sure how long I have driven with this malfunction since the light that indicates the air bag being on or off is off to the side and in yellow rather than red to show that something is wrong. I had to pay for the diagnosis and repair. This should have been covered by an extended warranty by the manufacturer. The car only had 67k miles, being 7 years old.
